Billing and Insurance

At Endo Surgi Center of Old Bridge, we are committed to maintaining close working relationships with most local insurance plans. We proudly participate with the following companies:

When you register, we will ask you to sign an “assignment of benefits form,” or ABN, to authorize payment directly to this facility. We will file the claim to your insurance carrier after your procedure. If the insurance company sends payment to your home address, it is your responsibility to send the payment onward to the Center.

As per the Out-of-Network Consumer Protection, Transparency, Cost Containment and Accountability Act, , it is your right as a patient to request information or have questions answered regarding fee or payment policy.

If you have any questions regarding your bill, please contact our business office between the hours of 8:30 a.m. and 4:30 p.m., Monday through Friday at 732-679-8808 for Facility billing, and 732-952-5533 for Anesthesia billing.

Important Billing Information

Physicians services provided in the Facility are not included in the Facility’s charges.
Physicians who provide services in the facility may or may not participate with the same health plans as the Facility; Patients should check with the physician arranging for the Facility services to determine the health benefits in which the physicians participate.
Patients should contact their carrier for further consultations on those costs.
Facility Standard Charges are available for services provided.

Pursuant to this new legislation, notice is required to be provided by all health care providers, physicians, and facilities.
